Do: The second bolt is reached from the front of the extruder through the access hole. Drive it, then tighten both to snug.
Check
Push and pull the extruder: no movement relative to the carriage. Both bolts tight.
What you're looking at
The second bolt again comes from the front, through the access hole. With both in, extruder and carriage are one rigid body, which stops the toolhead rotating under belt acceleration and printing a smeared corner.
